Tazemetostat is an oral targeted anticancer medicine that inhibits an enzyme called EZH2 (enhancer of zeste homolog 2).

It is an anti-cancer targeted therapy used to treat adults and pediatric patients aged 16 and older with metastatic or locally advanced epithelioid sarcoma that is not eligible for complete resection and adults with relapsed or refractory follicular lymphoma whose tumors are EZH2 mutation-positive or who have no satisfactory alternative treatment options. Tazemetostat works by selectively inhibiting EZH2 (enhancer of zeste homolog 2), a histone methyltransferase enzyme that is often overactive in certain cancers and drives tumor growth by silencing tumor-suppressor genes. By blocking EZH2, this medicine restores normal gene expression to slow or stop cancer cell proliferation. It is taken orally under the supervision of a qualified oncologist.

Epithelioid sarcoma

It is indicated for:

• Adults and patients 16 years and older
• With metastatic or locally advanced epithelioid sarcoma
• When the tumor is not eligible for complete surgical removal.

Relapsed or refractory follicular lymphoma

Tazemetostat is indicated for adults with relapsed or refractory follicular lymphoma:

• Whose tumors are EZH2 mutation-positive, detected using an FDA-approved test, and
• Who have received at least two previous systemic therapies.

It is also indicated for adults with relapsed/refractory follicular lymphoma who have no satisfactory alternative treatment options.

Tazemetostat selectively inhibits EZH2, including certain gain-of-function EZH2 mutations.

EZH2 is the catalytic component of the PRC2 (Polycomb Repressive Complex 2). It adds methyl groups to histone H3, particularly at lysine 27, which can suppress the expression of certain genes.

By inhibiting EZH2, tazemetostat can alter abnormal gene regulation and reduce proliferation of certain cancer cells.

In laboratory models, tazemetostat has demonstrated greater antiproliferative effects in lymphoma cells carrying EZH2 mutations.

In simple terms:

Abnormal EZH2 activity → abnormal gene regulation → cancer cell growth

Tazemetostat → EZH2 inhibition → altered gene regulation → reduced cancer cell proliferation

The FDA-recommended dose is 800 mg orally twice daily

This corresponds to:

Four 200-mg tablets in the morning + four 200-mg tablets in the evening.

It may be taken:

• With or without food
• Approximately 12 hours apart
• At the same times each day
• Until disease progression or unacceptable toxicity, unless the oncologist changes or stops treatment.

Missed dose

Serious:

Secondary malignancies—particularly important

One of the most important warnings associated with tazemetostat is an increased risk of secondary cancers.

Reported secondary malignancies include:

• Myelodysplastic syndrome (MDS)
• Acute myeloid leukemia (AML)
• B-cell acute lymphoblastic leukemia (B-ALL)
• T-cell lymphoblastic lymphoma (T-LBL)

Long-term monitoring is recommended.

Patients should report symptoms such as

• Persistent fatigue
• Easy bruising
• Fever
• Bone pain
• Pallor/paleness

These symptoms do not necessarily mean secondary cancer, but they warrant medical assessment.

Tazemetostat is metabolized by CYP3A, so medicines that strongly affect CYP3A can significantly alter its blood concentration.

Strong or moderate CYP3A inhibitors:

These can increase tazemetostat levels and potentially increase toxicity.

Examples include certain:

• Antifungal medicines
• Antibiotics
• Antiviral medicines
• Other CYP3A-inhibiting medicines

The FDA recommends avoiding these combinations where possible. If unavoidable, the tazemetostat dose may need to be reduced.

Always inform the oncologist about all prescription medicines, OTC medicines, vitamins, and herbal supplements.

The marketed product is supplied in bottles with a desiccant.

Keep the medicine:
