Ripretinib is an oral targeted anticancer medicine belonging to the class of kinase inhibitors. It is used primarily to treat adults with advanced gastrointestinal stromal tumor (GIST) after treatment with three or more previous kinase inhibitors, including imatinib.

Ripretinib works by inhibiting KIT and PDGFRA kinase signaling, including several mutations that can contribute to the growth and survival of GIST cells.

Important: 50 mg is the individual tablet strength; the standard recommended dose is generally 150 mg once daily, equivalent to three 50 mg tablets.

Dose reductions or modifications may be made by the treating physician based on tolerability and drug interactions.

• Advanced gastrointestinal stromal tumor (GIST) in adults
• Patients who have previously received treatment with 3 or more kinase inhibitors, including imatinib.

It should be used under the supervision of an oncologist experienced in the treatment of cancer.

Ripretinib inhibits signaling through KIT and PDGFRA, important drivers of many GISTs.

By blocking these kinase pathways, ripretinib can:

• Slow or inhibit growth of GIST cells
• Help control tumor progression
• Prolong progression-free survival in appropriately selected patients
• Potentially reduce or stabilize tumor burden

It is particularly designed to inhibit a broad range of KIT and PDGFRA mutations that may develop during the course of GIST treatment.

Take ripretinib exactly as prescribed by your oncologist.

The usual recommended dosage is 150 mg once daily.

This corresponds to three 50 mg tablets once daily. It may be taken with or without food.

• Take it at approximately the same time each day.
• Swallow the tablets whole.
• Do not crush, chew, or split the tablets.
• Do not change the dose or discontinue treatment without your oncologist's instruction.

If you miss a dose

• If you remember the missed dose and it is less than 8 hours from your usual dosing time, take it as soon as possible.
• If 8 hours or more have passed, skip the missed dose and take the next dose at the normal time.
• Do not take two doses together.

If you vomit

Do not take another dose. Take the next dose at the regularly scheduled time.

Common:

• Hair loss (alopecia)
• Fatigue
• Nausea
• Abdominal pain
• Constipation
• Diarrhea
• Muscle pain (myalgia)
• Reduced appetite
• Vomiting
• Hand-foot syndrome / palmar-plantar erythrodysesthesia
• Headache
• Weight loss
• Muscle spasms
• Mouth sores
• Dry or itchy skin
• Numbness or tingling of the extremities

Not everyone experiences these effects, and their severity can vary.

Serious:

Hand-foot syndrome

Ripretinib can cause palmar-plantar erythrodysesthesia syndrome, characterized by:

• Redness
• Swelling
• Pain
• Burning
• Blistering
• Peeling or bleeding of the skin on the palms or soles

Contact your healthcare professional promptly if these symptoms develop.

New skin cancers

• Ripretinib may increase the risk of certain new primary skin cancers, including cutaneous squamous cell carcinoma and melanoma.
• Regular skin examinations are recommended during treatment, and patients should report new or changing skin lesions promptly.

High blood pressure

• Ripretinib can cause or worsen hypertension.
• Blood pressure should be monitored during treatment, and uncontrolled hypertension should be addressed before starting treatment.

Heart problems

Ripretinib can cause cardiac dysfunction, including reduced left ventricular ejection fraction.

Patients may require assessment of heart function before and during treatment, depending on their clinical circumstances.

Seek medical attention for:

• Shortness of breath
• Swelling of the legs/ankles
• Unusual tiredness
• Chest discomfort
• Rapid or irregular heartbeat

Impaired wound healing

Ripretinib can interfere with wound healing. Treatment generally needs to be withheld around major elective surgery according to the prescribing physician's instructions.

Photosensitivity

• Ripretinib may increase sensitivity to sunlight.
• Patients should limit unnecessary/prolonged UV exposure and use protective clothing and sunscreen.

Ripretinib is metabolized primarily through CYP3A pathways, so medicines that strongly affect CYP3A can alter ripretinib exposure.

Medicines that may interact include certain:

• Antiepileptic medicines
• Antibiotics/antifungals
• HIV medicines
• Rifampin and related medicines
• Herbal products such as St. John's wort

Patients should provide their doctor/pharmacist with a complete list of prescription medicines, OTC medicines, vitamins, and herbal supplements before starting ripretinib.

At 20°C–25°C (68°F–77°F)

Temperature excursions between 15°C and 30°C (59°F–86°F) are permitted.

Keep the tablets in the original container.

Keep the desiccant in the container to protect the medicine from moisture and light.

Close the container securely after each use.

Keep out of reach of children.

Do not transfer the tablets to another container unless advised by a pharmacist.

• Take ripretinib exactly as prescribed and do not alter the dose yourself.
• Regularly monitor blood pressure during treatment.
• Have regular skin examinations, particularly for new or changing lesions.
• Inform your oncologist if you develop redness, pain, blistering, or peeling of the hands or feet.
• Tell your doctor about heart disease or previous cardiac problems before treatment.
• Inform your healthcare team before undergoing surgery or dental procedures.
• Protect your skin from excessive sunlight because ripretinib can cause photosensitivity.
• Tell your doctor about all medicines, supplements, and herbal products you take.
• Avoid starting St. John's wort or other supplements without discussing them with your healthcare professional.
• Do not stop ripretinib without consulting your oncologist.
